Mirror Box Treatment

 

What is mirror box treatment?

Mirror box treatment is a form of therapy used on patients who have an affected limb. The patient simply places there hand in a box installed with mirrors. The reflection of the limb in the mirror fools the brain into thinking painless movement of the affected limb has occurred. This helps to encourage treatment of the limb and help patents restore any injuries.

How Does It Work?

Mirror Setup: The patient places the intact limb (such as the hand or arm) into one side of a box that has a mirror placed diagonally inside, so when they look into the mirror, it appears as though they are seeing the reflection of their intact limb where the missing limb would be.

Visual Illusion: When the patient looks into the mirror, they see the reflection of the intact limb superimposed over the space where the missing limb would be. This creates a visual illusion that the missing limb is still there and moving normally.

Mirror Therapy Exercises: The patient is then instructed to perform various movements with their intact limb while watching the mirror reflection. For example, if they are experiencing phantom hand pain, they may be asked to open and close their intact hand while watching the mirror reflection, which creates the illusion that their missing hand is also moving.

Brain Reorganization: The theory behind mirror box therapy is based on the concept of neuroplasticity, which is the brain’s ability to reorganize itself and form new neural connections in response to experience. By providing visual feedback that the missing limb is still present and moving normally, mirror therapy aims to “trick” the brain into reorganizing itself and reducing the perception of pain associated with the missing limb.

Gradual Improvement: Over time, with regular practice of mirror therapy exercises, many patients report a reduction in phantom limb pain. The exact mechanisms underlying the effectiveness of mirror therapy are still not fully understood, but it is believed to involve both sensory and motor pathways in the brain.

 

What Patients Would Use Mirror Box Therapy?

Although it can be used for anyone suffering with injuries to a limb or recovering from surgery, it is most commonly used on Stroke victims. A common side effect of a stroke is limb weakness and/or paralysis, commonly in the left hand/arm. This can lead to everyday tasks such as washing, eating, drinking etc a lot more difficult and reduce patients independence.

A study conducted on stroke victims found that there was a 40% reduction in the muscles pain after completing a course of Mirror Box Therapy. The therapy course is commonly ongoing for a few months, however, studies have shown that after around 5 sessions, each last on average 40 minutes, there was a decrease in pain.

What is bilirubin?

Bilirubin is a yellowish pigment that forms as a byproduct of the breakdown of red blood cells in the body. It’s produced in the liver and then excreted from the body in bile. Bilirubin is responsible for the yellow colour of bruises and the yellow discoloration of jaundice. Normally, it’s processed by the liver and eliminated from the body through the digestive system. However, if there’s an issue with the liver or the bile ducts, bilirubin can build up in the blood, leading to jaundice and other health problems.

How is bilirubin measured?

Bilirubin levels in the blood can be measured through a blood test called a bilirubin test. There are two main types of bilirubin measured in this test:

Total bilirubin: This measures the combined levels of both unconjugated (indirect) and conjugated (direct) bilirubin in the blood.

Direct bilirubin: Also known as conjugated bilirubin, this specifically measures the amount of bilirubin that has been processed by the liver and is ready for excretion.

The test is often ordered if someone shows symptoms of liver or bile duct disorders, or if they have symptoms of jaundice, such as yellowing of the skin or eyes. The results of the bilirubin test help healthcare providers diagnose and monitor various liver and bile duct conditions.

How is jaundice treated?

The treatment for jaundice depends on the underlying cause. In many cases, jaundice resolves on its own as the underlying condition improves. However, if the cause is more severe or requires intervention, various treatments may be recommended. Here are some common approaches:

Treating the underlying cause: If jaundice is a symptom of an underlying condition such as hepatitis, gallstones, or liver disease, treating that condition is the primary focus. This might involve medications, lifestyle changes, or other interventions.

Phototherapy: For newborns with jaundice (neonatal jaundice), phototherapy is often used. Special lights help break down bilirubin in the skin so that it can be excreted from the body more easily.

Blood transfusion: In severe cases of jaundice, particularly in newborns with high levels of bilirubin that pose a risk of brain damage (kernicterus), a blood transfusion may be necessary to replace the baby’s blood with fresh blood that has lower levels of bilirubin.

Medications: In certain cases, medications may be prescribed to help reduce bilirubin levels or to treat the underlying cause of jaundice. For example, if jaundice is caused by a blockage in the bile ducts, medications or procedures may be used to remove the blockage.

Liver transplant: In cases of severe liver disease or failure, where other treatments have been ineffective, a liver transplant may be considered as a last resort.

Explaining Some of Our Latest Product Enquiries

In the past few months, we have been receiving heaps of new products enquiries; can you supply this? What about this? We need these as well! A lot of the products we have been able to supply, and a lot of them are things that you may never have even heard of, let alone know how to use!

First of all, we received a call from a healthcare professional enquiring for some medical oral syringes. Now, although his doesn’t seem out of the ordinary, you can get these from supermarkets, they weren’t just any syringes. The sought after syringes needed to be ENFIT Compliant. In fewer words, these means that they needed to work with other pieces of equipment and not come with a cap. The cap on syringes has proved to be rather dangerous and it is now standard procedure to opt for syringes without the cap.  Enfit Compliant syringes are available in a variety of sizes, from 1ml up to 20ml and are used in various hospital departments and units for all sorts of different procedures.

Following on from this, we also received a separate request for Medicine Bungs. Now at first, some of our team were clueless as to what these were. However, they were all surprised to learn they knew exactly what a medicine bung was. Have you ever been prescribed a liquid medication that has a rubber stop around the rim of the bottle? Well, if you have, the rubber stopper is actually called a medicine bung.  Although, these are often only given to parents with young children. The bung is designed to help with the administration of liquid medications through a syringe.  So, if you have a fussy child, who refuses to accept medicine without a fight, try a medicine bung!

Another product we received a call about in the last few weeks is Sterile Latex Gynaecology Gloves. These are not your average gloves. These are designed for gynaecology staff who may be doing frequent cervical examinations and helping with pregnant women. They extend up the forearm until the elbow for added protection. Although it is not common to find latex gloves in a hospital, these gloves are made from latex purposefully. Latex is a thicker material, offering more protection to both women and healthcare professionals are using them/ However, it is important that all parties confirm any latex allergies before moving ahead with these gloves as latex is common allergy.

 How is this better than the previously used trays?

Well whilst being able to withstand temperatures up to 130 (C), this product can also be sterilised with Autoclave, Ethylene Oxide or Hydrogen Plasma further enhancing sterilising to the max extent. All this is possible thanks to the product being manufactured from high quality polypropylene which also prevents any rips or tears in packaging during the sterilisation process.

Stainless Steel or Polypropylene?

Stainless steel and Polypropylene are both highly rated in the medical industry with plastic being intended for single use. However polypropylene is a stronger plastic that due to contrary belief can withstand the same temperatures as stainless steel however they do tend to absorb fluids and occasionally become disfigured. The key issue between these two is the shelf life as plastic has a shelf life of roughly 4 months compared to stainless steels 10 years. Whilst more expensive than the plastic it is a huge investment as one steel tray’s shelf life outlives 30 plastic ones (allegedly). Overall, the plastic ones are intended for single use however if you’re not satisfied, we can offer the alternative stainless-steel trays.

The government aims is to ‘ensure bereaved parents feel supported through their grief and recognise their loss, acknowledging their pain and ensuring they feel heard’. However, the certificates will not be compulsory – it remains the choice of all parents and caregivers to manage their period and time of a loss, however they see fit.

The government aim to extend the backdate of parents who have experienced loss beyond 2018, however, at the moment, this is the furthest back a death can be registered and a certificate claimed.

Further Updates On Commercial Shipping Delays!

In recent days, alarm bells surrounding commercial shipping vessels were raised yet again. In December 2023, a shipping vessel was captured and taken-over by Somali pirates when passing through the Indian ocean. This takeover included keeping all of the vessels crew hostage on board until they were later rescued in March 2024.  The takeover was the first of its kind since 2017 when the last report of Somali pirates was recorded. Somali pirates used to be a major concern for ships travelling along the Indian ocean as takeovers and attacks were rife. In 2011, the amount of takeovers by the Somali pirates hit its peak, but eventually died down.  The aim of the pirate groups is to capture a ‘mother ship’, a ship that is big enough for them to travel great distances across the oceans. Once they have this, they can takeover even larger vessels.  Despite this recent attack by the pirates, it is thought that they do not pose as a great threat as of yet, however it is imperative that commercial vessels are cautious, specifically when progressing through the Red Sea.

Meanwhile, the Red Sea is becoming increasingly more dangerous, with the Houthis mounting attacks on ships. Now, they harbor bigger ambitions and seem to have acquired deadlier weapons. So far, they have disrupted only one maritime route, but now they want to go further. They aim to stop trade altogether and extend their operations all the way to the Indian Ocean. They want to prevent ships from taking alternative routes, including the one via the Cape of Good Hope. How will they do this? Reports say there’s a new weapon in the Houthi arsenal: the hypersonic missile. With better weapons and bigger ambitions, what do the Houthis plan to do next? Our next report will tell you. The Houthis have become a dreaded name in the world of maritime trade. The striking of ships has spooked businesses, affected supply chains, and compelled ships to take longer routes to avoid them. Despite global condemnation, the Houthis remain undeterred. They already control the Red Sea, and now they have a bigger target. They aim to increase their area of influence. “Our main battle is to prevent ships associated with the Israeli enemy not only from passing through the Arabian Sea, the Red Sea, and the Gulf of Aden, but also, with the grace and assistance of Allah Almighty, we aim to prevent their crossing through the Indian Ocean and from South Africa towards the Cape of Good Hope,” they declare. Global trade heavily relies on the Suez Canal, which means passing through the Red Sea. It’s the shortest route between Europe and Asia, which is why ships prefer it. Twelve percent of global trade passes through the Red Sea every year, worth over $1 trillion. Once the Houthis started targeting this route, ships were forced to take a detour, sailing all the way around Africa through the Cape of Good Hope. Of course, it’s a much longer route, adding almost 3,500 nautical miles, which means over a week of extra sailing. However, it’s safer, which is why ships are taking this route. And now, the Houthis want to extend their attacks. They aim to ensure that ships can’t pass through the Cape of Good Hope either. They don’t just want to disrupt maritime trade; they want to stop it altogether. The Houthis primarily operate in the Gulf of Aden, but they are now planning to expand operations to the Indian Ocean, aiming to intercept any ship before it reaches the Cape of Good Hope.

The Yemeni rebels have lofty ambitions, and they seem to be acquiring new weapons to fuel them, such as the hypersonic missile. Russian state media claims that the Houthis have obtained one hypersonic missile and have successfully tested it. If true, this would put the Houthis in an elite club; only a handful of nations have this technology, such as the US, Russia, and China. Of course, these claims cannot be verified, but if true, it would give them a significant advantage. Hypersonic weapons fly at speeds higher than Mach 5, with the one allegedly possessed by the Houthis supposed to be Mach 8—eight times the speed of sound. Not only that, but the Houthis intend to manufacture even more weapons, giving them the power to attack more targets, posing a formidable challenge for Israel and the US. This means only one thing: the conflict in West Asia is spilling over. The Houthis started with a warning, followed by attacks. Countries tried to deter them, but nothing has stopped them yet. Now, with better weapons and bigger ambitions, they aim to expand from the Gulf of Aden to the Indian Ocean. The battle at the high seas has just begun and will likely escalate in the coming days.

Did you know that we supply custom creative design services?

We have a creative design team dedicated to creating bespoke, custom designs for customers looking for something more personalised. This can be done on pretty much anything you want (don’t hold us to that…).

So what have we worked on in the past? Well, we have worked on plenty of design for our customers. More recently, we have created custom meeting reminder cards. Cytology Brushes

What are Cytology Brushes?

Cytology Brushes are used in various medical procedures but particularly in the field of Cytology (the branches of biology and medicine concerned with the structure and function of plant and animal cells).

They are used to obtain Epithelial cells from the bronchial mucosa (Epithelial cells cover the skin, body cavities and blood vessels and the Bronchial Mucosa covers the surface of the epithelium).

How are the Cytology brushes designed?

The Cytology brushes consists of a handle and bristles. The bristles can be made from different materials such as nylon or polyester and are often arranged in a spiral or circular pattern to facilitate cell collection.

The handle is designed to be an easy manipulation for healthcare professionals during procedures.

Use of Cytology Brushes?

One of the areas that the brushes come in handy for, is one of the well-known uses in Gynaecology, particularly during Pap smears.  

Pap smears are routine screenings for Cervical Cancer where the brushes are employed to collect cells from the cervix. The cells are then examined under a microscope where they will detect any abnormalities that may indicate the presence of Cancer.

Another way where the Cytology brushes are used, is in endoscopic procedure where healthcare professionals used specialised Endoscopes (Endoscope is an inspection instrument to look deep into the body) to examine the internal structures of organs. The cytology brushes come In play when they collect the cell samples from the lining of the organs.

Furthermore, there is another way as to how the brushes can be used and that is in the field of Pulmonology (Pulmonology is a medical specialty that deals with diseases involving the respiratory tract). The brush will pass through a bronchoscope to collect the cell samples from the respiratory tract.

Lastly, they can also be used in medical research to collect cellular material to develop knowledge on creating new treatments.

Niche Healthcare received a query from a Medical Device store which distribute medical devices in Romania. They had a tender project for Colposcopes (a surgical instrument used to examine the vagina and the cervix of the uterus). Within the project, they were asked to add 2500pcs of the Cytology brushes and they contacted Niche Healthcare for a price on 2500 units.

What is Menopause?

A Menopause is something that happens to a woman naturally between the ages of around 45 to 55 years old. It is where your period will stop due to lower hormone levels.

In some cases, it can happen earlier.

What is Andropause?

With Menopause which effects only women, there is Andropause which affects Men.

Andropause is something that happens when there is a decrease with the sexual satisfaction or a decline with the general well-being due to lower testosterone levels. There is no fixed age for Andropause, but I can start to occur from the ages of 30. Another thing, Andropause doesn’t affect all Men like Menopause does with Women.

Treatments for the Menopause:

Not all women need treatment with the Menopause but the women who seem to have the worse symptoms can receive some types of treatments.

One treatment is a hormone replacement therapy which includes tablets, skin patches, gels and implants which help to relieve the menopausal symptoms as it replaces oestrogen.

Another treatment is different types of vaginal oestrogen cream, lubricants, moisturisers to help the vaginal dryness which some ladies will seem to have.

Cognitive behavioural therapy is the third treatment which can be given which is a talking therapy to help with the anxiety.

Lastly, eating healthy can be a suggestion to the lifestyle change as it can benefit some of the menopausal symptoms.

Treatments for Andropause:

Unlike Menopause which have multiple treatments which can be offered, with the Andropause, only comes one treatment which is a testosterone replacement to correct the hormone deficiency. This can be given as a gel or injection.

However, there are also some natural ways to try reverse Andropause which Is to exercise regularly, eat healthy, reduce alcohol consumption and consume Zinc or Vitamin D

Side effects of Andropause and the Menopause:

Like every treatment there are always side effects. The side effects for Menopause treatment can be headaches, breast pain/tenderness, vaginal bleeding/spotting, nausea, depression, leg cramps, itching and diarrhoea.  

Side effects for the Andropause treatment can be a decrease in energy and muscle strength, decreased libido, mood changed and cognitive function.

PPE, Personal Protective Equipment is protective clothing which a person should wear when carrying out a job which could potentially be hazardous.

Why is it important to wear PPE in the NHS?

PPE within the NHS is required due to harmful substances such as chemicals or infectious agents. The PPE, helps to prevent the transmission between the staff and patients.

One example of the above, will be when Covid happened, and we had to wear things such as Gloves or Masks to help prevent the spread of the virus. Wearing the equipment helps control the risks of spreading an infection.

The basic PPE uniform, consists of the following:

Full Face Shield – Face shields are Personal Protective Equipment devices that are used to help protect the facial area associated with the mucous membranes, such as the Eyes, Nose and Mouth. This part of the PPE uniform shields your face from any splashes, sprays and spatter of body fluids.

FFP3/Fluid Resistant Surgical Face Mask – The face covering helps to avoid any spreading of a virus by stopping any respiratory droplets which contains viral particles. It helps manage more suspected patients.

Nitrile (Non-Latex) Medical Disposable Gloves – The use of Medical Gloves, if recommended as part of standard precautions to reduce the risk of contamination of healthcare workers’ hands with blood and other body fluids including contact with non-intact skin and mucous membranes.

Healthcare Suit/Scrubs – Healthcare scrubs prevent infections and maintain hygiene in a healthcare setting, making them important for the healthcare professionals. Unlike traditional tunic, makers of scrubs, design then for use exclusively in the hospital environment.

 

What could happen without the use of PPE?

Without PPE, exposure could lead to injuries or illnesses. In certain industries, people are required to wear PPE as a standard daily thing. For example workforces, such as construction, mining, chemical and pharmaceutical industries are all those among the NHS which are required to wear these to protect themselves so they are safe whilst carrying out there job.

If PPE, isn’t used when required to, it could lead to immediate and long-term risks which threaten yours and people surrounding you.

UK Faces Growing Medicine Shortages

Why is there a shortage?

There are thought to be multiple reasons why the UK is facing medicinal shortages. However, one major reason is presumed to be the backlash of the Brexit referendum which has (and continues to) cause the cost of living to increase. Despite spiking costs, the NHS were also left unable to source medicines from manufacturers abroad. Annual analysis suggests that over the past 2 years (2022 – 2024) the percentage of shortages has increased by 100%. There are currently 96 medicines in short supply with a further 10 suspected to be added any time soon. However, the shortages reached an all-time high at the end of October 2023, when 111 medicines were in shortage across UK pharmacies.

Moreover, ongoing wars between Israel and Ukraine are adding to cost and stocking issues, causing prices to skyrocket outside of the NHS’ agreed annual spenditure for branded medications.

Are there alternatives available?

For some of the medicines there are alternatives available. This could be lesser/higher dosses or completely different prescriptions. However, not all medicines have efficient alternatives available. It is thought that unlicensed imports of medicines could potentially be the answer to the shortages. Where there are different shortages for each medication, there are estimated period f which it is presumed medicines will be unavailable. For example, glucagon-like peptide-1 receptor agonists will be difficult to acquire throughout the whole f 2024 and there is no certainty of annual stocks.

 

How is this affecting UK Pharmacies?

Pharmacies across the UK are dealing with the ongoing struggle and having to repeatedly inform patients that they cannot fulfil prescriptions and medications for long term illnesses. It is suspected that pharmacies are spending on average an extra 11 hours a week attempting to find the short supply medications for their patients wherever they can. 87% of pharmacies agreed that that their patients mental and physical health was being put at risk daily with all of the ongoing shortages.

What Is Happening On The Red Sea?

The Houthis, who control much of Yemen, have attacked dozens of ships in the major waterway since November, disrupting international maritime trade and raising concern about the impact on the global economy.

Due to the attacks by Houthi rebels on the Red Sea, there are increasing delays on stocks and delivery times. Alternative shipping routes are now adding an additional 9+ days on the arrival of stocks from overseas to the UK, as well as increasing costs by thousands of pounds. As a result of this, hundreds of products, including healthcare consumables, are falling in short supply.  We are working diligently to ensure we can get orders out and keep stock levels steady. An estimated 30% of the world’s traded consumer goods are shipped through the Suez route. Higher transport costs obviously raise costs for shippers, but how they are affected depends on specific contracts although surcharges may hit them even if they have term-contracts.

How Is This Affecting Business Around the Country?

Not only is cargo shipping being disrupted, but cargo is being destroyed whilst on route. This is causing major stock issues for countries all over hoping to re-stock this new year and get orders out to their customers. Businesses having to choose alternate shipping options, including air and rail freight are dealing with increased costs and prolonged lead times on deliveries.

In order to go around the Red Sea, shipping vessels are expected to travel around the Cape of Good Hope, Africa. This alternate route allows commercial ships another route into European countries. However, this will add 10,00km+ to each journey it completed, which equates to 10+ added days of travelling. these added kilometres, as well as added travel days, are costing companies upwards of £1000 on top of their original shipping costs. 

To avoid these costs and delays, air freight has become in demand. As a result of this, freight airlines are now facing increased demand for air freighting, with no choice but to increase there own prices to cope with demands.

NeoSAFE- Advanced Infant Feeding Maturity Device.

Infant feeding maturity is a developmental process that involves the baby’s ability to feed safely and effectively. It is an important aspect of their early growth and well-being: also known as suck-swallow-breathe coordination. The device allows for measurements to be taken on infants whilst feeding to assess their feeding habits and ensure they are consuming enough milk. This is crucial, especially on pre-term and jaundice infants.

This device is to be put in place, to understand the development of an infant by being able to gather feeding data analysis at the touch of a button.

Analysis of the NeoSAFE Audit programme:

The way the babies were compared, were by using 52 preterm infants of between 27 and 36 weeks with a control group of 42 healthy full-term infants. The feeding performance was based on how the babies swallowed the milk during 2-miniute audio recordings. The following variables were generated for the overall evaluation by using the NeoSAFE device;

Methods & results of using the NeoSAFE device:

With the help of a sensor placed on the hyoid region under the chin and the diaphragm of the chest during feeding, the device is able to measure the number of total swallows.

The number of recorded swallows results in term and late preterm infants, was significantly higher than the swallowing data that was obtained by the device. It was made clear, that there was no obvious difference between the two groups in the evaluation of IASw which is one of the parameters which determine the risk of aspiration. Once the results are examined by ultrasonography (a technique which uses echoes of ultrasound pulses to delimit objects or areas of different density in the body) it was then observed that the MaxRs data is similar to the Neo SAFE device.

On the 28th of November 2023, we received an Enquiry from an Australian NHS health board called Queensland Health.

Characteristics of Motor Neuron Disease:

Columbia University – Lancet Enquiry

Last week, we received an enquiry from a resident of Medical Genetics in Pontificia Universidad Javeriana, who requested our Niche Heel stick safety Lancets.  The purpose of the enquiry was to perform a clinical trial to change to sample by which to screen newborns. In Columbia, they are currently screening for newborn genetical diseases using cord blood sampling from birth whereas in the UK and other countries, this is tested via Heel Stick. Therefore, the university were partaking in a trial where they tested the effect of both cord blood sampling and heel stick sampling.

Cord blood sampling refers to the collection of blood from the umbilical cord of a newborn baby immediately after birth. The process of cord blood sampling is relatively simple and painless. After the baby is born and the umbilical cord is clamped and cut, a healthcare professional can collect the blood from the remaining umbilical cord using a needle and a collection bag or vial. This blood is then processed and stored in a cord blood bank for potential future use.

A heel stick test, also known as a heel prick or newborn screening, is a common procedure performed on newborns shortly after birth to screen for certain genetic, metabolic, and congenital disorders. The test involves pricking the baby’s heel and collecting a small amount of blood onto a special filter paper. This blood sample is then sent to a laboratory for analysis. The heel stick test is typically done within the first few days of a baby’s life, often before they leave the hospital.

Today, was another great day to enhance our growing work, development to push our healthcare innovation range products to our Irish hospitals. We obtained today, our first order going to the Neonatal department (Ulster Hospital – Belfast)  , we have been working closely with the Ward Manager to get our new range of products to this hospital, which was founded in 1872.

The Ulster Hospital is the major acute hospital for the Trust and delivers the full range of acute services for the population.
